At Revive Health and Wellness, we provide you with access to an online portal where you can fill out and complete your intake forms from the convenience of your own home or office. Your forms will be accessible to you through your portal anytime. If you need to make changes to your first appointment we kindly ask for a minimum 48 hours notice to allow other patients to come in during that time.
